Rhinoplasty is a plastic surgery operation performed by making changes in the bone and cartilage structures of the nose shape in this structure.

Nasal aesthetics, also called nasoplasty and rhinoplasty, is one of the most frequently applied aesthetic operations. Rhinoplasty is performed to eliminate the deformities of the nose that are congenital, occur later, or occur due to trauma. While deformity can be corrected in some cases only because it causes visual discomfort, it may also need to be corrected because it causes problems such as respiratory disorders in most people.

Regardless of the reason for the aesthetic operation, the primary goal in rhinoplasty is to make the nasal airway work optimally at the end of the operation. During the operation, one or more of the operations such as reshaping the nose (reconstruction), removing deformities or changing the shape of a part of it can be applied.

In people considering rhinoplasty, whether the nasal tissues are suitable for aesthetic operation should be checked during the examination to be performed by the surgeon. Since the slightest intervention produces very significant results, surgeries related to rhinoplasty must be performed by a surgeon who is an expert in the field.

Rhinoplasty, which can be applied to both men and women, is a complicated operation. It takes a period of 6 to 12 months to fully achieve the expected result. This process varies according to the size of the operation in the nose, the structure of the tissues in the nose or the severity of the problem. After surgery, minor corrections may be necessary in some patients to achieve the desired shape.

What is nasal tip aesthetics?

The applications performed before and after the surgery are almost the same as other rhinoplasty surgeries. The operation time is shorter and all procedures are completed in a period of 30-60 minutes. Patients are usually discharged on the same day. Recovery time is also shorter compared to rhinoplasty surgeries.

Why is rhinoplasty performed?

Rhinoplasty can be performed for functional or cosmetic reasons. One of the reasons for performing rhinoplasty is the presence of breathing problems due to a congenital or subsequently developed deformity in the nose. The deformity that causes respiratory problems may be a visible defect, or it may be a defect in the inner part of the nose that cannot be noticed from the outside. The second reason is that the person is uncomfortable with the appearance of his nose and wants to have a different-looking nose.

Conditions such as a curvature or protrusion in the bony part of the nose, deformities observed in the cartilage structure, excessively large nose, and asymmetry in the nasal structure are among the reasons that require rhinoplasty surgery. Personal preferences play a role in operations performed for aesthetic purposes.

While some people are very happy with the appearance of their nose despite having a very large or protruding nose, some people may prefer a different nose image despite having a normal nose structure. As a result, rhinoplasty operations are very common operations that can be performed due to health problems or personal preferences.

Many changes can be made in the structure of the nose with surgical intervention:

How is rhinoplasty done?

In rhinoplasty surgeries, the pre-operative period is as important as the operation itself. In interviews with the physician, first of all, it is evaluated whether the patient is a suitable candidate for the aesthetic operation. At the same time, the physician and the patient should be able to fully explain their expectations and possible results through mutual exchange of ideas.

The physician evaluates the medical history of the patient to be operated on, his/her discomforts and current medications. In the presence of health problems that may cause excessive bleeding, such as hemophilia, cosmetic nose surgery may not be appropriate.

In the physical examination performed by the physician after the medical history is taken, the patient's nasal skin, internal and external structures are examined and the changes to be made are planned. Physicians may also refer to various blood analyzes and biochemical laboratory tests to decide the suitability of the patient before surgery.

Nasal photos of patients who are planned to have surgery can be taken. These photographs can be effective both in illuminating the expectations after the surgery and in guiding the physician during the operation. Another issue that patients should pay attention to is that non-steroidal anti-inflammatory drugs such as ibuprofen and aspirin should not be used 2 weeks before and after the surgery. These drugs can have a bleeding-increasing effect by negatively affecting the coagulation functions. It should not be forgotten how important it is for your physicians to know all the medicines and supplements you use for your health and recovery.

Another behavior that can negatively affect the surgery and the postoperative period is tobacco use. Cigarette smoke both slows down the healing process and causes contraction in blood vessels with nicotine in its content, resulting in a decrease in oxygen and blood supply to the tissue during the healing period. It is recommended to stop tobacco use in the pre- and post-operative period for a healthy recovery process.

Nose aesthetic surgery is a type of operation that is mostly performed under general anesthesia. In simple rhinoplasty operations that require only small touches, local anesthesia can also be applied according to the surgeon's preference. While the operation takes 2 hours on average, a rhinoplasty operation usually takes 1 day, together with the pre-operative preparations and post-operative applications and observation periods.

There are 2 different rhinoplasty techniques that are widely preferred. In the first of these, open rhinoplasty, a small incision is made between the nostrils and the surgeon's angle of view is widened thanks to this incision. Generally, the open rhinoplasty technique is preferred in people with advanced nasal deformity or who have had a different nasal aesthetic operation before. The biggest advantage of the open technique is that the viewing angle is wider. However, the healing process is longer and the problems such as edema and bruising after the operation are larger.

In closed rhinoplasty, which is another technique, the incision is adjusted to remain inside the nostrils. The surgeon's field of view is less, but the recovery time is shorter and there are no stitches on the nose after the operation. The closed rhinoplasty technique is generally preferred for correction of smaller deformities.

Although the face is the most striking region of the person at first glance, the correction of deformities in this region is very important in terms of aesthetics and psychology. Who cannot have rhinoplasty?

There are various health problems that are considered as contraindications for rhinoplasty (conditions that should not be applied):

Body Dysmorphic Disorder

Body dysmorphic disorder is a psychiatric disorder and is a state of extreme prejudice about one's image. Due to this extreme level of prejudice, the person may be influenced by even small flaws that are very difficult to notice from the outside, triggering the formation of sociological and psychiatric conditions such as socialization problems, low quality of life, depression and suicidal tendencies. It is appropriate for the physicians who will perform rhinoplasty to know these patients, who cannot be satisfied with the postoperative results, and to mediate in the realization of the necessary psychiatric evaluation.

Obstructive Sleep Apnea

Obstructive sleep apnea is defined as cessation of breathing due to recurrent obstruction of the airways during sleep. Physicians should be more careful because of the high probability of developing undesirable conditions before surgery in patients with this disease. If this condition is present in patients who need rhinoplasty, the patient's breathing should be supported with devices such as CPAP in order to minimize the risk.

Bleeding Disorders

Care should be taken as various undesirable situations may occur after rhinoplasty in people whose blood coagulation is adversely affected. In order to prevent this situation, it is necessary to question any excessive bleeding or bruising or the use of supplements and vitamins that are effective in clot formation during the pre-operative interview with the patient. Nose bleeding is one of the most common complications after rhinoplasty. In people with bleeding disorders, nosebleeds may be excessive and it may be necessary to intervene in the bleeding vessel with endoscopic or angiographic methods.

As a general approach, people who have undergone rhinoplasty surgery before and are not satisfied with the results should wait at least 1 year before making a new evaluation. At the end of this period, the final results of the previous operation are evaluated and a second correction operation can be applied when necessary.

What are the dos and don'ts after rhinoplasty?

Pain usually occurs after the surgery, and therefore painkillers are administered to the patient before the effect of the anesthesia wears off. Afterwards, the drugs recommended by your doctor should be used without interruption. It is generally not recommended to stand up for a period of 4-6 hours after surgery. It is normal to experience problems such as dizziness and blackout when you first stand up. For this reason, it is recommended to seek help from a relative.

It is normal to have blood leaking from the nose both during and after the surgery. This bleeding can be swallowed unconsciously, causing nausea and vomiting. For this reason, bloody vomit or black stools may be observed the next day due to blood. 6 hours after the operation, if there is no nausea, water can be drunk. Juicy foods such as soup and ayran can be started to be consumed little by little.

For the first few days, a liquid diet is usually recommended. It is normal for swelling and bruises to occur under the eyes, around the nose and in certain parts of the face, and will gradually fade away. In order to prevent bleeding, the pillow should be kept high and water should not be drawn into the nose for the first 15 days or blown. It is normal for the nose to remain blocked in this process.

It is not recommended to wear glasses for several months (in some cases 6 months) so that the targeted shape can be fully achieved. Physical activities that require high effort such as jogging, swimming, excessively vigorous chewing, laughing, smiling or other gestures with the participation of many facial muscles, and vigorous brushing of teeth are examples of other behaviors that patients should avoid after surgery.

Another important issue that people should pay attention to after the operation is exposure to sunlight. Care should be taken as permanent color changes may occur in and around the nose skin after excessive sun exposure.

Keeping the head elevated is an effective practice as well as bleeding. Depending on whether the suture material used during the operation is absorbable or non-absorbable, it may be necessary to consult your physician again 1 week after the operation to have the sutures removed.

How long is the recovery period for rhinoplasty?

Although the recovery period in patients undergoing rhinoplasty varies according to the size of the operation, patients can usually get up and walk around within 4-6 hours, and eat after 6 hours if there is no nausea and vomiting. In some cases, patients are discharged on the same day, and sometimes they are discharged the next day after being kept under observation (observation) for 1 night.

At the end of the surgery, tampons are usually placed in the nose. These tampons are removed 1-2 days later, and the mold placed on the back of the nose is removed approximately 1 week after the operation. If no complications occur after the surgery, patients can usually return to work within a period of 1 week to 10 days. It is normal for swelling and bruising to occur after the operation, and they lighten over time and decrease to a level that can be covered with make-up within 1 week-10 days.

After 1 month after the operation, 75-80% of the edema disappears. The targeted shape and full recovery are achieved one year after the operation. During this process, it is normal to see edema in the nose area that occurs in the morning and disappears in the following hours.
